Pre-delivery
Birth Certificate
A representative from the Health Information Department will call you with questions for your baby’s birth certificate. The information needed is lengthy, including over 30 questions. At the time of the interview, parents should be able to provide their newborn’s full name, social security number for both mother and father, and the mother’s city of birth. The birth certificate must be signed before your discharge from the hospital. A copy of the baby’s footprints is provided, compliments of the hospital.
